Please try to submit this request form well in advance of the desired completion date. We do our best to fulfill all requests in a timely manner, however please note that depending on the nature of your request turnover time may vary.

• We do not provide or pay wages for support persons, but upon request they may accompany an individual to a program or event free of charge.
• If a document or service is free in its original format, you will not be charged a fee for requesting an alternate format or communication support.
• If a document or service comes at a cost in its original format, you may be charged that same fee when requesting an alternate format or communication support.
