Snow Angels Volunteer Nomination Form

We want to recognize the generous "Snow Angels" or volunteers in our community who are helping out a neighbour during this 2020 / 2021 winter season.

If you have gotten help from someone to clear the snow from your driveway and / or walkway you can fill out this nomination form to enter that person in a monthly draw to win a City of Belleville sweater or toque.

You will need to know the person's name and at least one method of contact, such as a phone number, email, or mailing address. We encourage you to let the person know you would like to enter them in this draw and ask if it is okay that you provide their name and a method of contact.

Personal information will only be used for the purpose of selecting and contacting the winners each month. After a prize has been awarded any personal information will be deleted. At the end of the 2020 / 2021 winter season all entries and personal information will be deleted.
