Integrity Commissioner Complaint Form

Please complete this form to submit either a Code of Conduct Complaint or a Conflict of Interest Complaint. 

This form also includes a commissioning section that must be completed before submission. Commissioning will be provided free of charge however there is a fee of $150.00 for submitting the application. The commissioning section will appear in the printed version of the form for signing and completion by the appropriate official.
